The external consultation process ran between September - November 2015. A Consultation Summary has been published which describes the comments received in relation to the application form and the manner in which they have addressed in the new version.

The submissions made by external parties are also published below. As the consultation process related to a range of documents, including but not limited to the application form, the comments contained in the submissions may not be directly applicable to the application form.
